🚨 Very Important Notice

Please make sure your child has a lunch box and a water bottle every day at camp.

📋 Parental Consent and Medical Authorization

As the parent (or legal guardian) of the above-named student, I understand that my student will be participating in a number of activities conducted by SFGC, which carry a degree of risk. I consent for my student to participate in these activities.

I understand and agree to the following:

  • My child/youth is physically fit and has the necessary skills to safely participate in these activities
  • I give consent for my student to travel to and from events in transportation provided by volunteer drivers
  • As parent and/or legal guardian, I remain legally responsible for any personal actions taken by my child
  • I agree to release, indemnify, and hold harmless SFGC, its directors, officers, representatives, members, agents, guests, volunteers, and employees from any and all claims, including but not limited to bodily injury or property damage claims

PHOTOS & MEDIA:

All participants in programs may be photographed or recorded for promotional or social media use, including but not limited to Facebook, Instagram, program websites, and other similar outlets.

MEDICAL AUTHORIZATION:

I understand that SFGC will attempt to notify me in case of a medical emergency involving my student. If the church cannot reach me, I authorize the staff to hire a doctor or healthcare professional to provide necessary treatment. I will pay for any medical expenses incurred.
